Reports and Documents

  • Reports for ALL Exchange Students

    Help us help you - completing your monthly reports and sharing your experiences, both good and bad, is essential to help us run this program.

    Monthly Activity Report is due by the 5th of each month. Receipt of these reports is essential to our being able to approve proposed travel for both Inbound and Outbound students.

    Indicident Report - medical emergency/accident or other serious inicident is first reported by the student. As necessary, a Rotarian will complete the Rotary International Incident Report.

    Travel Authorization - Inbound Students must initiate this process. Emails will be sent to your natural parents, host parents, host club to confirm and authorize the trip. DO NOT purchase travel prior to authorization except in emergency situations. If leaving the US, you MUST have your DS2019 signed by the Responsible Officer prior to exiting the US - you will be denied re-entry without that signature. OUTBOUND students - you are bound by you HOST district travel rules.

    The following application is due by email on NOVEMBER 1. No Exceptions to this deadline are considered, please plan accordingly!
